The spacer spaces the PTO gearbox back from the tractor about 4" or so allowing the use of a readily available 1 1/8" to 1 3/8" splined adapter. The adapters are usually available at TSC and other farm stores.

This set up was once commonly used to allow one to use the 1 1/8" PTO pulley on a tractor with a 1 3/8" PTO shaft. I do not believe that the spacers are commercially available anymore and I have myself bought 3 or 4 of them over the years.
