Assuming a reasonably close ratio (1:3 or less) building a low speed PTO chain drive is almost trivial and relatively inexpensive. Once you get much above 1:3 sprocket size is going to complicate things considerably. Aside from the shaft mounting it doesn't matter whether you run it as a reducer or increaser - the chain and sprockets don't care.
